Male Breast Reduction - Gynecomastia

Gynaecomastia in Greek means 'women like breasts'
It typically starts when a boy transits to pubertal phase from childhood. Normally all boys would undergo this phase of growth in breasts. But if it persists for more than two years it becomes a cause of concern. Enlarged male breasts causes emotional discomfort and impairs self-confidence. When male breasts become too large and start showing through clothing, it becomes an emotional challenge for such boys to participate in most of the sports activities like swimming. There is preponderance and a strong association in children with obesity, though most of the hormonal levels may be normal. In some cases they can also have an associated hypothyroidism status. There could also be other causes such as medications or gender ambiguity.

Grades of Enlargement

Depending upon severity of enlargement it is classified in to three grades
Grade I :only minimal breast tissue enlargement
Grade II :Breast tissue enlargement with minimal excess skin
Grade III :Breast tissue enlargement with excess skin

Gynaecomastia Is Characterized By:

Excess localized fat
Excess glandular tissue development
A combination of both excess fat and glandular tissue
Gynaecomastia may present unilaterally (one breast) or bilaterally (both breasts)

Pre -Operation Work Up

Anaesthesia: General / local Anaesthesia with Sedation
Incision: For Liposuction alone a small nick is sufficient to remove excess fat. In case Excision is decided upon, generally periareolar, semicircular incision is given.
Procedure: Gynaecomastia is commonly treated with both liposuction and excision

Liposuction:

In cases where gynaecomastia is primarily the result of excess fatty tissue, liposuction techniques alone may be used.

Excision:

Excision techniques are recommended where glandular breast tissue or excess skin must be removed to correct gynaecomastia.

Results and Recovery: After surgery, dressings or bandages will be applied to your incisions and an elastic bandage or support garment may be used to minimize swelling and support your new chest contour as it heals. Activities: You will be able to perform routine activity in 3-4 days and will be allowed to go for strenuous exercises in 3-4 weeks time

Why do I have breast enlargement even though I am a boy or a man?
This is a result of hormonal manipulations that occur in our body when any child transits through puberty to reach adulthood. A prolonged preponderance of female sex hormone can result in persistence of pubertal enlargement of breasts. It can further increase in size if one puts on weight. This happens due to associated fat deposition.

Is something wrong in Identity of my gender?
It is rare. Although in some cases of ambiguous genitalia or chromosomal abnormality such as 47XXY (Klinefelter Syndrome), Gynaecomastia is one of the manifestations.

Will I have problem in producing babies?
Only if there is associated infertility such as in chromosomal abnormality or urogenital abnormalities

Is it curable?
Surgery is the best and the only option to completely cure this condition. The recent advances in surgery include liposuction for fat removal and creating a plane and glandular excision by peri-areolar approach. In severe grades where there is excessive skin enlargement Plastic surgeons are trained to take care of that by various methods

Is there any other option like drugs or injections for treating this?
You may be advised by doctors or friends or some websites to try out pills such as Gynexin etc. Kindly do your homework well before trying out these quick fixes. Scientifically, it is impossible to reverse the growth of breast tissue. Using these pills, you may also run the risk of losing some hair in bargain.

Will there be Scars or telltale evidence that I have undergone surgery?
Liposuction entails a small incision that leaves a scar of 0.5 cm or less. Gland excision leaves typically a peri areolar scar which being at the junction of areola and the normal skin produces an insignificant mark after some time

What are the risks associated with surgery?
Surgical risks are similar to that involved in any other surgery. Specifically common adverse results can be a little delay in wound healing, seromas and not so pleasing chest contours.

Will it recur?
If done properly by a well-trained plastic surgeon or a general surgeon, it should not. Though if one gains some weight, in the long run it can manifest as chest fullness that is called as pseudo-gynaecomastia and not the true enlargement of a gland

What is the Post surgery care?
One has to wear compression garments for three weeks. Other sport activities will also be restricted for almost three to four weeks
